Today, the provision of health care and social services is extremely important, especially to people, who have special needs or who cannot afford paying for health care services they need. In such a situation, the assistance of state agencies as well as non-for-profit organizations may be needed. However, the effectiveness of services delivered by these agencies and organizations depends on the proper model they use in the service delivery. In this respect, it is possible to distinguish tow basic models, which are currently applied: case-management service delivery and federated service delivery organizations.
Case-management service delivery is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s health needs through communication and available resources to promote quality cost effective outcomes (Chambers and Wedel, 2006). In this respect, it should be said that the case-management service delivery is particularly effective in relation to people, who have serious health problems, which may be aggravated by their negative psychological state. For instance, many people may have difficulties with the transportation and movement that prevents from attending health care institutions and receiving professional health care assistance.
As for the federated service delivery organizations, they are more concerned with the maintenance of the health care services and their provision to people. They mainly focus on the effective management of health care organizations and development of strategies to improve the quality of health care services delivered to patients. They are very important for the development of health care system and provision of health care services of the high quality.
Some organizations, like ROSS-service delivery models elderly/disabled, are quite successful in delivery health care services to people with special needs, while they apply case-management service delivery.
