The article that was chosen for analysis has a title “Evidence-Based Practice: Use of the Ventilator Bundle to Prevent Ventilator-Associated Pneumonia” and it was written by the group of physicians and researches. I’d like to write research article critique and dwell on the main research questions that seemed the most interesting for me. The problem of conduct of patients with respiratory insufficiency and different kinds of pneumonias continues to remain the very important issue of the day until now. This problem is actual nowadays and we should find the best method to solve it as soon as possible.
An article chosen for our analysis like every article has own structure and all facts and research approaches are presented in certain order. It has a purpose, method, results, and conclusion. I’d like to describe its content in the beginning of my work and only then when all readers will plunge into the problem I will pay attention on its specific details and use specific examples. I think that such research critique will form an attitude to the article and show to the readers that article is really important and its data useful for its readers. The purpose of it is short, well-formulated and directed on care nurses’ knowledge about the ventilator bundle use for the prevention ventilator-associated pneumonia examination. The method of research supposes testing for nurses in two stages and receiving results before and after implementation of concrete actions. It is necessary not only to state and then discuss specific information, but also use it in practice and receive results, I want to emphasize that it is necessary to receive positive results. Saying about results we should mark out that after the concrete education sessions, the nurses received not only new knowledge but also new skills that will help them to be more effective in their further work with ill people and help them to become healthy. In acknowledgement of my words it is necessary to quote the part of article with the results of test and with the help of numbers show that education was really effective. The results are following: “The areas of most significant improvement were elevation of the head of the bed (P < .001), charting of the elevation of the head of the bed (P=.009), oral care (P=.009), checking of the nasogastric tube for residual volume (P=.008), washing of hands before contact with patients (P<.001), and limiting the wearing of rings (P<.001) and nail polish (P=.04).” Apparent improvement proved that such education is important part of nurse activity and it increases the average quality of patient care in several times. The conclusion of the article summarizes that education sessions are necessary for medical personnel and I also want to add that they are necessary not only for nurses, but in my opinion for all medical personnel because there are many innovations appear from day to day to day and physicians should be informed about them in the first rate. All adopted methods of the work have a significant effect on interested in them participants and clinical practice will be improved in such way.
The beginning of the article focuses on the question of ventilator-associated pneumonia (VAP) and its research importance, because only studying of the hot issues of our life we can rescue ill people. It is stated that the American Association of Critical-Care Nurses (AACN) worked out methods how to reduce the incidence of VAP and propose for use steps based on specific points of the best-practice guidelines for people suffering from above mentioned disease and receiving mechanical ventilation. According to the article these steps, often called the “ventilator bundle” unite “the following guideline from the Centers for Disease Control and Prevention (CDC) for preventing nosocomial pneumonia:
Elevation of the head of the bed (HOB) to 30º to 45º unless medically contraindicated;
Continuous removal of subglottic secretions;
Change of ventilator circuit no more often than every 48 hours, and
Washing hands before and after contact with each patient.”
As we understand from quotation all these steps are easy, feasible, but their results will help to prevent VAP and even need no additional costs for patients’ treatment. It is marked out in the article that “there are no recommendations about gastric residual checking or oral care to prevent ventilator-associated pneumonia”, but in my opinion it is necessary information and these recommendations should be formulated and confirmed by their implementation in life in chosen area of medicine.
I can say that there are many statistical data and results of different studies presented in the article and people who are interested in them could receive not only theoretical recommendations, but also check their truth using data used in the article. Data are gathered in several tables and its allow to understand them in better way and not only read analyze of the table data, but also to make own its analysis and be sure that presented methods and recommendations are really working and it is only necessary to familiarize personnel wit them and use recommend methods in own work.
Thus, based on all above stated it is necessary to conclude that many problems connected with VAP prevention are not researched and discussed until our days, but at the same rate many basic aspects are implemented in practice and have good results. The data pictured in the differed tables and studies discussed in the body of researched article show us that it is necessary to teach personnel to be more effective in their practice and everyday use elementary rules of VAP prevention that are easy and feasibly. I want to repeat that teaching of medical personnel today, as never, becomes actual. And it is a not an obvious attempt to follow the fashion, it is a necessary condition for successful work in the conditions of permanent changes and developed competition environment. Education sessions help personnel quickly to adapt oneself to the changing environment and work with it most efficiency. But nevertheless many leaders have unsolved questions for what and why to teach personnel and article that was chosen for our discussion can be also used as a guideline for leaders with concrete recommendations and examples from real life.
For the conclusion of my paper I want to say that accordance of personnel standards, produced to the profession, and also internal standards of company are necessary moment in medical sphere. For example, accordance to the standards of co-operating with patients during their treatment. Exactly on training and knowledge sessions critical care nurse or another personnel have possibility to get complete information about that, how it is necessary to operate and what to talk, and also there is possibility to make attempt apply received knowledge in practice, to get a feed-back and fasten the got result in role games with colleagues. When an employee appears with a problem one on one, without the preliminary teaching for him it is enough difficult to choose correct tactic, and in process of work with the patients there are no right on mistake for medical personnel, because it can cost life to person who need help right now and right here.
